Fractured metatarsal had a metal plate and 5 pins installed. I work out side and noticed that my left foot will start heating up with shoes an socks on standing in the sun. I also have tingling in my foot i assume nerves. The last x-ray shows my ankle and foot are ok....but that does not explain the swolling, and color changes. Also, there were two small spots of arthritis starting. Which would make sense on my foot tightening up on me while I am sitting for a long time.

ANSWER

Hi Richard,

I have said the following on my site numerous times and that is we as doctors can never put the human body (in this case the foot), back together as well as mother nature originally built us.
So yes, you had a metatarsal fracture and your surgeon determined that you needed a plate and screws. They were inserted, the fractured essentially healed and now you are walking again, but with some difficulty.
You do not mention how long ago the surgery was performed, but in general, if it were performed within the last 12 months it would not surprise me that you are still getting swelling and some discoloration as it actually takes bone upwards of a year to remodel. The fact that you stand a lot does not help matters, but it is what it is. You state your left foot heats up, that is probably from the inflammation, but keep in mind, it the plate is close to the surface of the skin, theoretically, the plate could heat up if exposed directly to the sun, if you were wearing sandals for example.
The tingling, I would imagine, would either be from the swelling putting pressure on one of the local nerves, or the nerve itself might have been irritated during surgery, which can happen.
Have you voiced your concerns to your surgeon as that is the person who is best suited to evaluate your condition and progress?
Keep in mind, that if your doctor determines it is the hardware in your foot that is causing most of your problems, at some point down the road, the hardware could be removed.

Well just to update you...i still have issues..mainly with pain, and tingling, also noticed that hot water seems alot hotter with the left foot then it does the rest of my body.

I had a nerve test and it shows everything is ok. The latest issue is walking all day then all of a sudden -- the pain was so much I could not take another step -- I had to sit down and wait for the pain to stop. So far it has only happened once. The nerve doctor said from the issues I described to him sounded like chronic pain syndrome. But, the surgeon, and my primary doctor have not said anything about it.

October will be 1 yr since the surgery.

Yes I have mentions to him and shown his photos that I take everytime this happens. That way I have dates. All he does it X-ray and says everything looks good. This last time he X-ray my foot and ankle he said everything was good but he saw two spots where arthritis is starting to form.

As far as my job -- I work a bucket truck - climbing in and out - and walking and lifting. Which as you said is not helping things. The Dr. had to put one pin sideways to hold the two bones together then pur 4 pins straight down in from the top into the bone. I will have to find out how close to the top the plate is. I wear sneakers or work boots all the time.

Dear Dr. Mitnick, The orthotics arrived four days ago and I slipped them into my shoes immediately. I was skeptical as to the usefulness of the item, they really didn't look very exotic. I have to say though, after using them for just four days, I have experienced grand relief from my foot pain. Even the very first day, I was able to do a lot of work while on my feet with at least a 75% reduction of pain. It has only gotten better every day, and I go nowhere without my shoes with the orthotics. I had been experiencing extreme heel and sole pain for about six months and had to take extended breaks off my feet many times a day as well as regular doses of Ibuprofen. Since getting the orthotics, my life has returned to normal and I feel good again. Just wanted to say thanks for the recommendation for a very effective item, I had no idea what a change this item could affect.

Dr. Mitnick, Thank you, you were 100% correct. The pain finally brought me to the ER. I spent 8 days in the hospital. The Doppler you spoke of was able to show that there was no pulse in that foot. This was an arterial clot that split and traveled throughout my leg. My leg was almost amputated. I am in rough shape but have all my parts intact!! You certainly know what you are talking about. After reading what you had to say, I called the doctor's office and was able to get in and see him the same day as my injury. Toe was x-rayed and luckily, it is not broken or fractured. Very badly bruised and will probably lose the toe nail. And although my toe and toe nail are still very black and blue and very sore, they ARE both starting to feel a little better.
